Mesenchymal stem cells (MSC) are being widely studied for their potential in tissue engineering and regenerative medicine. However, recent evidence suggests that the beneficial effects of MSCs may manifest with them release extracellular vesicles (EV); usually does not require the administration of MSC.
This evidence, particularly from pre-clinical in vitro and in vivo, suggesting that MSC-EVS may indicate a substantial therapeutic properties in various pathophysiological conditions, has the potential to restore a variety of damaged or diseased tissues and organs. These benefits of MSC EVS apparently found, regardless of the origin of the anatomy or body fluids from MSC (and including for example, bone marrow, adipose tissue, umbilical cord, urine, etc.).
In addition, early indications are that the beneficial effects of MSC-EVS can be further improved by modifying the way in which the MSC donor are cultured (eg, in hypoxic compared with normoxic conditions, 3D compared with the format of the culture 2D) and / or if EVs are then bio engineered (eg, loaded with certain). So far, several clinical trials in humans of MSC-EVS has been done and the question unanswered whether a heterogeneous population of EVs profitable or specific sub-populations, how best we can culture and scale-up production of MSC-EV and isolation for clinical utility, and in what format they should be given.
However, as reviewed here, there is now substantial evidence to support the use of MSC-EVS in tissue engineering and regenerative medicine and further research to establish how best to utilize this approach to social and economic benefits justified.
